Accident Investigation Training

 

Course Aim:

This is a one day course designed to provide the course participants with the appropriate knowledge to investigate, document and report on accidents or dangerous occurrences in the workplace to their employer.

Course Objectives:

On completion of this course, participants will be able to:

  • List the legal requirements relevant to the investigation of accidents in their workplace.
  • Evaluate and investigate accidents/dangerous occurrences.
  • Understand and complete IR1 & IR3 forms.
  • Document, record and produce reports on in-house incidents/accidents.
  • Be aware if/when further knowledge, skills or experience are necessary to investigate accidents or dangerous occurrences.
  • Recommend appropriate actions to prevent a re-occurrence of the accident.

Course Programme:

  • Legislation relevant to your workplace.
  • Overview of “ Accident Icebergs/Triangles”, “ Accident Proneness” and “Accident Causation”.
  • Resources required for accident investigation.
  • Interviews and interview techniques.
  • Securing the area.
  • The importance of: mapping, identifying, photographing, documenting and recording data on file.
  • Discussion on completion of the HSA, IR1 & IR3 forms

Training Methods:

The course takes place in a classroom situation with multi-media presentations, video, flipcharts, discussions etc. Trainees participate in, and interrogate, case investigations as part of the learning process. Course handouts help trainees recall detail for their workplace.

Who Should Attend:

Those persons in organisations who are deemed responsible for accident investigation and reporting.

  • Maximum of 12 participants.
  • 1 Day duration
